VOLUNTEER SIGN-UP FORM (2010)
Open to DPT I, DPT II, DPT III students and Preceptors

DIRECTIONS
1. Register on our registration site. You will then be redirected to this site to sign-up for one of the available clinic dates.
2. Click the checkbox next to each clinic date on which you would like to volunteer.
3. At the bottom of the page, enter the email address you used to register with us. This is what we will use to look up your information to determine whether you are a DPT I/DPT II/DPT III or Preceptor.
4. Click Submit!
5. After you click Submit, this page will automatically refresh. Your name will appear by all the dates you have successfully signed up for.

If you run into any problems, please notify Web Master.
If you cannot make it on a date you've previously signed up for, please let Rebecca Elbare know.
