VOLUNTEER SIGN-UP FORM (2008)
Open to DPT I, DPT II, DPT III students and Preceptors

DIRECTIONS
1. Register on our registration site. You will then be redirected to this site to sign-up for one of the available clinic dates.
2. Once you have registered your information, peruse the volunteer openings for the clinic dates below. For each date we need:
2 DPT I students
2 DPT II/DPT III students
1 Preceptor

Alternates: 2 DPT II/DPT III students, 1 Preceptor
3. Click the checkbox next to each clinic date on which you would like to volunteer.
4. At the bottom of the page, enter the email address you used to register with us. This is what we will use to look up your information to determine whether you are a DPT I, DPT II/DPT III, or Preceptor.

Alternate spots are only available once the original 2 DPT I I/DPT I I I and 1 Preceptor spots are filled.
To sign up as an alternate for a clinic date where the original spots have been filled, just follow the same directions and your name will appear under the Alternates column.
5. Click Submit!
6. After you click Submit, this page will automatically refresh. Your name will appear by all the dates you have successfully signed up for.

If you run into any problems, please notify Jane Pak.
If you cannot make it on a date you've previously signed up for, please let Chris Fussell know.
